Who we are
Pet ESA Service operates petesaservice.com and arranges ESA and PSD evaluations with independent licensed mental health professionals. Questions about this policy go to hello@petesaservice.com.
Information you give us
When you complete the assessment, we collect your name, email address, phone number if you choose to give it, the state where you will be during the evaluation, confirmation that you are 18 or older, your answers to the wellbeing questions, information about your animals and your housing situation, and anything you choose to write in the free text boxes. Some of this is health information, and we collect it only with your express consent.
When you pay, Stripe collects your card and billing details on its own secure page. We receive the payment status, amount, a payment reference and the email you used, never your full card number. When you contact us, we receive your name, email and message.
How we use it
We use assessment information to arrange and complete your evaluation, to let your clinician review your answers, to contact you about your order, to deliver your letter if one is issued, to answer housing provider verification requests that you authorize, to process refunds, and to keep the records that licensing rules and tax law require.
We do not sell personal information, we do not share health information for advertising, and we do not run advertising or social media tracking pixels on this website.
Who we share it with
Your evaluating clinician. Our hosting provider, which stores the website and the submitted forms in a private folder that is not publicly accessible. Our email provider, which delivers notifications and your letter. Stripe, for payments and refunds. A housing provider or college only when you authorize verification, and then only to confirm authenticity. We may disclose information when the law requires it.
How long we keep it
Assessment records and evaluation documents are kept while your evaluation is active and afterward for as long as clinical record keeping, licensing and tax rules require. Contact messages are kept for as long as needed to answer them and for ordinary business records. Payment records are kept as tax and accounting rules require.
Security
Pages and forms are served over HTTPS. Submitted assessments are stored outside the public website folder with restricted access. Card data never touches our servers. No method of storage or transmission is perfectly secure, so please share only what your clinician needs.
Your choices and rights
You can ask to see, correct or delete your information, or withdraw consent for future processing, by emailing hello@petesaservice.com. We will verify your identity before acting. Some records cannot be deleted while clinical record keeping or legal obligations apply. Residents of states with consumer health data laws should also read our consumer health data notice.
Children
Our services are for adults 18 and older. We do not knowingly collect information from children.
Changes
We will post changes here with a new date. Material changes that affect how we use health information will be highlighted on this page before they take effect.
