Consumer health data notice

This notice explains how we handle consumer health data, including for residents of Washington, Nevada, Connecticut and other states with health data laws.

Categories of consumer health data we collect

Answers to the assessment about mood, anxiety, sleep, daily functioning, treatment history, diagnoses you choose to share, and how your animal helps you, plus related free text. We also collect identifiers that link to that data, such as your name, email, phone number and state.

Why we collect it

To arrange and complete the evaluation you request, to let a licensed clinician decide whether documentation is appropriate, to deliver any letter, to support verification you authorize, to handle refunds, and to keep required records. We collect it only with your consent, which you give on the assessment form.

Sources and sharing

We collect consumer health data directly from you. We share it with the evaluating clinician and with service providers that host our forms and deliver email on our behalf under confidentiality obligations. We share authenticity confirmation with a housing provider only when you authorize it. We do not sell consumer health data and do not use it for advertising.

Your rights

You may confirm whether we hold your consumer health data, access it, ask for a list of the third parties we shared it with, ask us to delete it, and withdraw consent for future collection and sharing. Email hello@petesaservice.com with the subject line "Health data request". We will respond within the period your state's law requires and explain any records we must keep. If we deny a request, you may appeal by replying to our decision, and you may contact your state attorney general.
